Everyone needs a place of calm to reflect and take stock especially in these times, But at LDCM work continues at pace as we strive to help all our clients take those daily steps that one after another bring them closer to their goals.
These steps often go unseen as a lot of what we do is very much ‘behind the scenes’ so I caught up with Liz Lambert, one of our most experienced case managers to ask her about what a day in case management looks like for her. I think it’s safe to say that busy doesn’t quite cover it, and I was amazed at the amount and scope of work that Liz handles every day with care and professionalism.
Hi Liz, how is this week shaping up for you?
“I’m Preparing for another busy week as CM at LDCM. This week I was carry out three Zoom MDT meetings with therapists, solicitors and clients. I will ensure clients are making progress with their goals and identify any needs that may need addressing. I will carry out two support worker supervisions online to ensure our staff feel well supported and are enabling the clients to reach their goals. I will introduce a new Physiotherapist to a client and ensure they are fully briefed on what the client is working towards. Finally, I will be carrying a Zoom meeting with a client, their Epilepsy nurse and the school nurses to discuss their Epilepsy care plan in school. Another full and satisfying week ahead!”
Great to hear from Liz and, as with all the team, her focus is on constantly helping the clients move toward their goals. Hoping to share some future insights from our team and how they are all going the extra mile to provide a truly special service to all our clients.
